The answer is simple and a bit creepy. For example you have a 20th great grandparent who is also your first cousin 20 times removed. Or you have a 15th great grandparent who is also your great uncle 16 times removed. Bluntly - inbreeding and a massive amount of it. There is no other solution. Pedigree Collapse = inbreeding.
